Domain names are charged separately. Online shops, booking integrations and extra pages are quoted separately. Hosting renewal and any ongoing services are confirmed in your quote. Design or marketing work is priced for the job: a new logo and a full content campaign are different things.

No, website care is optional. You can also ask for one-off changes, quoted as you need them.
